I have not encountered any meat labeled "enhanced" or anything like that so far. Should I presume that if it is not labeled that, especially if it is labeled fresh per USDA, it is not enhanced? I intend to ask a butcher next time, with some ribs or other meat in hand, whether they are enhanced or not and see what response I get.

I don't think they usually say enhanced. I think you'll see something like "10 percent solution". The solution is typically a salt solution and the percentage of salt runs about 10-15 percent.
